Below are the rebate application forms you and your contractor need to fill out completely. Equipment must be purchased and installed between January 1, 2012 and December 31, 2015 to be eligible. Upon receipt of your application, you can expect your rebate check in 6-8 weeks.
Rebate applications for equipment purchased prior to December 31, 2011 are no longer being accepted.
Residential Electric Rebate - Use this form for a rebate on an energy-efficient central air conditioning system, central air-source heat pump, electric heat pump water heater or programmable thermostat.
Residential Natural Gas Rebate - Use this form for a rebate on an energy-efficient furnace, water boiler, steam boiler or indirect hot water heater that uses natural gas.
Duct and Air Sealing Rebate - Use this form if you had blower door testing and air sealing and/or duct blasting and duct sealing performed by a Building Performance Institute (BPI) certified contractor.
Federal Tax Credit for Energy Efficiency Energy-efficient products or renewable energy systems installed before December 31, 2011 may also qualify for a Federal tax credit for 10% of cost up to $500 or a specific amount from $50-$300. Must be an existing home and your principal residence. Consult your tax advisor.
